Information

Testosterone is the principal androgen product produced from cholesterol. In men, it is responsible for the development of male genitalia and secondary sexual characteristics. In females, its main function is to produce estrogen. Free testosterone and testosterone bound to albumin consists of the bioavailable testosterone. Testosterone bound to sex hormone binding globulin (SHBG) is considered biologically inactive. Free testosterone consists of 1-2% of the total testosterone whereas testosterone bound to albumin makes up 20-40% and the inactive portion is 60-80% of the total testosterone.

Diagnostic Guidelines

Free testosterone (FT) is used as second-line test in conditions associated with altered SHBG concentration, or when total testosterone is at or near the lower limit of the normal range for patients with clinical features suggestive of androgen deficiency. Assessment of FT might improve the diagnosis of hyperandrogenism and PCOS in women.
